This form is to correct an inadvertent error on the part of the institution.
The Undergraduate Tuition and Fees Appeal process is available to Undergraduate students who feel they warrant an exception to the Withdrawal/Refund Policies as listed in the Class Schedule for the appealed semester due to extenuating circumstances such as severe medical illness, an employment situation beyond their control or the death of an immediate family member. Appeals are only accepted for charges incurred less than 1 years prior to the application for appeal.
The Graduate Tuition and Fees Appeal process is available to Graduate students who feel they warrant an exception to the Withdrawal/Refund Policies as listed in the Class Schedule for the appealed semester due to extenuating circumstances such as severe medical illness or the death of an immediate family member. The Appeal must be filed before the conclusion of the Semester following the Medical Emergency or Death.
This form is to be used only in the event that the online registration system failed to capture a transaction.

The Sponsorship Payment Program makes it easier and more convenient for employers and/or sponsors to pay for the educational costs of their beneficiaries. This mutual agreement between sponsors and MSU Denver allows for a direct billing option for payment of tuition and fees. To initiate the direct billing option, please complete and submit this form to Office of the Bursar.
Every year the school issues a 1098-T form for your tax purposes. In order to access your 1098-T online follow the directions in the 1098-T form.
If you miss the deadline to authorize your COF for a semester, there may be limited time to turn in a manual authorization form. After calling to see if you are still eligible for the manual COF, please fill out this form and either fax it in or hand deliver it ASAP.
Use this application if you are applying for a Work-Study job with either the Office of the Bursar, Perkins or the Cashiers Office.
